Since its formation on February 6, 1986, the Presidential Commission has undertaken a thorough investigation into the tragic Challenger accident. This report outlines our findings and offers crucial recommendations for your consideration. Our mission is not only to prevent a recurrence of the failures that led to this disaster but also to minimize risks in future space missions. Each member of the Commission shares in the profound grief felt across the nation following the loss of the seven courageous astronauts on January 28, 1986. As we move forward, our goal is to ensure the return to safe space flight and reaffirm Americas leadership in exploration. Honoring the memory of the Challenger crew means committing to these vital improvements.
